Proof of Delivery (POD) Template

Use this Proof of Delivery template to record when, where and in what condition freight was delivered, who received it and whether shortages or damage were noted. It is a working record, not automatic proof that every contractual requirement was met.

Ready-to-use template

Copy this structure and adapt it to the actual shipment and your company process.

Practical guide

Treat the template as a controlled working draft: fill only verified shipment data, keep references consistent across documents and retain the signed or approved final record when the workflow requires it.

Addresses, dates, times, quantities, weights, prices, reservations and document references should come from the order or actual event—not from memory or an old shipment.

How it works

Copy the structure, replace every placeholder with shipment-specific information, review the completed draft with the responsible party and use the required signed or official document in the real workflow.

When to use it

Use it at delivery when dispatch needs a consistent record of arrival, received quantity, condition, exceptions, receiver identity and signature or stamp.

What to check before dispatch

Check the customer’s required POD format and evidence: signed CMR, stamp, photos, timestamps, name, reference or portal upload may be mandatory. Record visible damage or shortage before documents are signed whenever the process requires it.

Worked example

Example: “10 EUR pallets received, packaging intact, 25.08.2026 10:07, received by J. Example” is useful only if those facts are actually verified and the required signature evidence is retained.

Common mistake

Common mistake: marking “delivered OK” despite a visible shortage or damage, or losing the signed document before it is sent back to dispatch.

FAQ — Proof of Delivery (POD) Template

Is this an official legal form?

No. It is a practical working template. Use the form, document or workflow required by the parties and applicable rules for the real shipment.

Can I use it for a real shipment?

Use the template only after adapting its fields to the actual shipment and workflow. Required parties, signatures, evidence and contractual details can differ.

What should I verify before finalizing it?

Before use, verify names, references, dates, quantities, addresses, signatures and any customer, carrier, site or legal requirements relevant to the movement.

What is the most common mistake?

The most common mistake is treating a generic template as complete. Missing shipment-specific instructions or evidence can matter more than the form itself.
